The standard super choice form has now been placed online so you can complete, print and sign for your employer.

It’s your chance to make sure your super stays in good hands, with an Industry SuperFund that is run only to profit members.

If you’re not sure or can’t remember if you’ve ever filled out a choice form, the best thing to do is to make sure your employer knows which fund you choose.

It’s simple; just complete this form and give it to your employer.

It will advise them to make contributions to your nominated Industry SuperFund.
